2. Make a check payable to Ladies Carnival Club, Inc. in the amount of            $100.00

3. Mail the completed application form and check to the mailing                      address listed below.

4.  Your application with accompanying payment will be reviewed for            acceptance.

5. Once the club reviews and approves  your application, you will be  placed on the waiting list until a spot opens up.
